What is a Certificate of Insurance and how do I submit it?

A Certificate of Insurance (COI) verifies a delivery company has an active insurance policy that would cover any potential damages to the residence that may occur during the delivery process. Some residences in large cities require a Certificate of Insurance prior to delivery.

If your residence requires a Certificate of Insurance, please contact your building's management team to obtain a copy of this form and submit it to our delivery partners using the form below. Please note, if your residence requires a COI, your order cannot be delivered until this documentation is received.
